Following the BCCDC’s COVID-19 Public Health Guidance for K-12 School Settings
Parents/guardians are not required to fill out a health assessment form before sending their child to Gurmat Center anymore. Parents/guardians will be required to self-assess their child for key symptoms of illness daily, before sending them to classes. These symptoms include:
Chills, fever (body temperature > 37.5°C), cough or worsening of chronic cough, shortness of breath, loss of sense of smell or taste, diarrhea, & nausea/vomiting.
Completion of this waiver confirms that as the primary caregiver, you will conduct a daily health assessment of your child BEFORE sending them to class. Families will only need to complete this waiver once per school year for each registered child.
Waiver submissions must be completed by Friday, November 13, 2020.
